We collaborate with each patient and with their families to set goals for the plan of care. Then our compassionate team works with patients and families to minimize symptoms, provide other medical care, coordinate other healthcare services, and maximize comfort & quality of life.
Our hospice team will visit your home when you call and on a schedule convenient for both the patient and family. In addition to medical care from nurses and therapists, our hospice aides help with personal care such as bathing and grooming. We will make sure your home is equipped with all the medical equipment you need, such as a hospital bed or home infusion devices. Our dedicated volunteers will be happy to visit with patients and give family members time for their other responsibilities. A medical social worker and hospice chaplain will be available for emotional and spiritual support. In addition, our team will help with documents and care planning such as power of attorney, DNR, DNI, a living will, and other advance directives. We also provide bereavement support for the families of our patients.
