With 86+ years of combined experience in delivering quality hospice care, the Prime Care Hospice staff will answer any and every question to help you decide if hospice is right for you or your loved one. Our staff is able to provide free consultations in the convenience of your home or location of your choosing to provide information or guidance for you or your loved one.

Hospice services are tailored to the needs of patients with life-limiting illnesses in an advanced stage. It is time for hospice after the doctors have offered all the curative treatments available or when a person’s goals shift away from aggressive treatments and toward comfort and quality of life. Generally, hospice is expected to last up to six months. We will coordinate with your doctor to certify hospice eligibility. Patients may discharge themselves from hospice care at any time should they want to pursue non-palliative treatments.
How to Begin Hospice
Anyone can make a hospice referral: a doctor, hospital discharge planner, patient’s caregiver, or even a patient him/herself. When gathering information or before enrolling, you may want to start with a no-obligation meeting at your home or a place of your choosing. When the time is right, just let us know. We will coordinate with your doctors and your insurance for you, and take care of everything.

In accordance with applicable federal and state law, we do not discriminate on the basis of race, color, national origin, religion, sex, gender identity, pregnancy, disability, age, medical condition (cancer-related), ancestry, marital status, citizenship, sexual orientation, or status as a Vietnam-era veteran or special disabled veteran. We also prohibit sexual harassment. This nondiscrimination policy covers employment, admission, and delivery of care.

If you need help filing a grievance, our Civil Rights Coordinator is available to assist you.
The availability and use of this grievance procedure do not prevent you from pursuing other legal or administrative remedies.
